To be better prepared for health emergencies at the church, we are asking all parishioners to complete the attached Emergency Information form. A copy of the form will be kept in a secure location with limited access, and would only be shared with responders in an emergency situation.

Participation is voluntary. Please omit any information you wish to keep confidential.
